Welcome to the TRACK questionnaire!
TRACK stands for Tripod Accident Causation Kit and is based on a reactive accident analysis method. It is checklist that structures the information found in an investigation and helps analyzing the facts. TRACK gives you the right indicators and divides your selections in eleven categories, the ‘Basic Risk Factors’ (BRF’s). This gives the investigator an idea of the categories in which causes of the incident fall into. TRACK also defines the ‘Preconditions’ and ‘Latent Failures’ of the incident. These can be used to draw the Tripod Beta diagram later in the analysis process. The outcome of the TRACK checklist (tab ‘TRACK Report’) is an overview of the ‘Preconditions’ and the ‘Latent Failures’ and a count of the BRF’s in a BRF overview graph.
